Provider: SPECIAL SERVICE FOR GROUPS - HOMELESS OUTREACH PROGRAM INTEGRATED CARE SYSTEM

The agency provides homeless support services for people in Los Angeles. Services include assessment for substance abuse for homeless adults and for street youth; case/care management for homeless individuals and families; general counseling services; outreach programs, including special programs for street youth and homeless women with children; mental health evaluations for homeless adults and for street youth; a re-entry program for women recently released from prison; and specialized information and referral for homeless individuals and families. There are no geographic restrictions.

Contract clinic services include outpatient health services for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary general medical care and dental care for eligible clients. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients. Contract clinic services are provided through T.H.E Clinic at the HOP/ICS location.

Mental health and substance abuse evaluations and treatment for adults and for street youth are provided at the Vermont Ave. location. APPLICATION PROCEDURE Walk in for service. FEES/PAYMENT SOURCE There are no fees for services.
