Provider: SOUTH CENTRAL FAMILY HEALTH CENTER - SCFHC MEDICAL AND DENTAL CARE CENTER

The program provides health services for people of all ages who live in South Los Angeles. The services are targeted, but not restricted to low-income people. The service area is SPA 6 which encompasses zip codes 90011, 90003, 90037, and 90044; and surrounding zip codes 90001, 90002, 90007, 90018, 90047,90062, however, there are no geographic restrictions.

Public/Private Partnership (PPP) provides outpatient health services for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary general medical care. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients.

CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.

General health services include cancer screenings for breast, cervix, colon and prostate cancer; cholesterol testing; diabetes screening and treatment, including retinal screenings; general physical examinations; high blood pressure screenings. The center also provides sexually transmitted disease testing and treatment and flu immunizations. The Patient Assistance Program provides financial assistance for the cost of medications.

Women's health services include obstetrics and gynecology, mammograms, family planning, pregnancy testing and a comprehensive perinatal program (CPSP). The perinatal program provides complete prenatal and postpartum care, education and nutrition classes and assessment and counseling.

Pediatric services include asthma control; Child Health and Disability Prevention (CHDP) exams; childhood immunizations; developmental assessments; general physical examinations, including medical and sports physicals; hearing screening; lead screening; vision screening; weight control classes.

The adolescent care program provides health and nutrition education, family planning services and specialized information and referral. APPLICATION PROCEDURE Call to apply or walk in for services. An appointment is preferred. Proof of income is required for some payment plans. FEES/PAYMENT SOURCE There are fixed fees; Medi-Cal, Healthy Families, and other private insurance are accepted. Eligible PPP patients pay no fees. An ability-to-pay plan is also available.
