Provider: EL PROYECTO DEL BARRIO - WINNETKA CENTER FOR A HEALTHY COMMUNITY

The agency provides health services for adults and youth in Los Angeles County and targets those who live in the San Fernando and San Gabriel Valleys. Services are provided from three locations, and are targeted, but not restricted, to Spanish-speaking people. Services include community clinic and contract clinic services: child health and disability prevention (CHDP) exams, general medical care, health education, immunizations, pediatrics, prenatal care, dental care and physical examinations. Applicants must meet Federal low-income guidelines; geographic restrictions apply for some services.

Contract clinic services include outpatient health services for uninsured, low-income Los Angeles County residents ages 19 to 64, including General Relief recipients. It provides primary general medical care. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients. People who have AIDS/HIV and who qualify for Healthy Way LA may receive primary care at the agency's Arleta clinic.

CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.

The clinic is a My Health LA Community Partner Clinic and provides application and enrollment in the My Health LA plan. My Health LA covers low income individuals (age 6 to adults) in Los Angeles County that do not have health insurance and cannot qualify for Medi-Cal.

Dental services are provided at one location: the Winnetka Center for A Healthy Community site, and are for people with no insurance and low income or poverty level. Services include cleaning, fillings and x-rays and are for existing patients only; new patients are not being accepted at this time. Emergency patients are seen, however the only services which will be provided are a prescription for pain medication and a referral to another agency.

The Arleta medical clinic provides primary health care for people of all ages. Services include: physical examinations, immunizations, obstetrics/gynecology, and hypertension. The family health care clinics in Canoga Park, Pacoima and Azusa provide the same services, plus health education and mammograms.
