Case/Care Management ( Alzheimer's Disease )

The program provides services for older adults 60 years and older who live, primarily in the Harbor City, Harbor Gateway, San Pedro, Wilmington area. Services include an in-home assistance registry and case/care management services. Geographic restrictions apply.
The program provides an in-home assessment for elderly clients who may be referred by family, hospitals or other social service agencies. Case managers assist clients to find appropriate care. There is also an in-home assistance registry of aides who can provide homemaking or personal care in the client's home, if necessary. The program provides transportation to and from the center to any of the congregate meal sites. APPLICATION PROCEDURE Call to apply for service. FEES/PAYMENT SOURCE There are fixed fees for the in-home aides and transportation services. Donations are suggested for case/care management services.
