Case/Care Management ( At Risk for Homelessness, Compulsive Hoarding, Older Adults )

The center provides a variety of services for older adults, 60 years and older, who live in the West Wilshire area of Los Angeles. Services include case/care management, certificates/forms assistance, congregate meals/nutrition sites, escort programs, friendly visiting, home delivered meals, recreational activities/sports, senior housing information and referral, senior centers, specialized information and referral, telephone reassurance, and welfare rights assistance. Age and income restrictions apply for some services; services are restricted to residents of the Hollywood-Wilshire area.

Case/care management includes housing location assistance; assistance in making plans for a senior to move to an assisted living or other type of facility; advocacy for public or private benefits; and assistance in completing forms, making appointments, or in writing or translating letters.

The Homeless Prevention Program provides case management for older adults age 55 and up in SPA's 4 and 5, who are living with a hoarding disorder and are at imminent risk of eviction. A mental health team will assist with declutter/cleanout services, harm reduction approaches, resolution of eviction issues and counseling.

The center also offers a variety of recreation and fitness activities, as well as occasional health screening and flu shots. APPLICATION PROCEDURE Call or walk in for information. Services are not denied because of an inability to pay.
