Provider: VENICE FAMILY CLINIC

The agency provides health services for low-income people of all ages who live in Los Angeles County, including people who are homeless. Services include AIDS/HIV clinics; child health and disability prevention exams; childhood immunizations; community clinics for low-income, homeless people, homeless young adults and homeless youth; contraception; contract clinics; flu vaccines; general medical care; HIV testing; obstetrics/gynecology; optometry; pediatrics; planning/coordinating/advisory groups; pregnancy testing; prenatal care; and sexually transmitted disease screening. Services are primarily offered to residents of the West Health District; income restrictions also apply.

Contract clinic services include outpatient health services for uninsured, low-income Los Angeles County residents ages 19 to 64, including General Relief recipients. It provides primary general medical care. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients. The clinic is a My Health LA Community Partner Clinic and provides application and enrollment in the My Health LA plan. My Health LA covers low income individuals (age 6 to adults) in Los Angeles County that do not have health insurance and cannot qualify for Medi-Cal.

The clinic receives additional support from the Ryan White Foundation. People who have AIDS/HIV and who qualify for My Health LA may receive primary care at the Rose Ave. location.

Child health and disability prevention exams (CHDP) provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.

Medical care includes diagnosis and treatment of primary medical problems, including HIV and AIDS, physical exams for children age 16 and younger, immunizations, pregnancy testing, family planning, dispensing of prescription medications and referral for laboratory and X-ray services at local hospitals. Additional services are provided through the agency's chronic care, women's, teen, and optometry clinics. The agency also provides a general medical clinic for homeless people and/or people who live in shelters in the area.

Bill's Clinic at SPY provides services for homeless youth and homeless young adults. The clinic is open one day out of the week and offers basic medical care such as care for basic wounds and infections. APPLICATION PROCEDURE Call the Central Appointment Line to schedule an appointment. (The Health Centers are only Accepting New Adult Patients who are assigned through Medi-Cal or Covered California and women who need prenatal care). Proof of address and income are required. Any documentation of address is accepted; income documentation may be GR letter, check stub or letter signed by employer or an income tax return. Homeless individuals may walk in for appointment assistance Monday through Friday, 10:00am to 11:00am. Bill's Clinic at SPY homeless youth and young adults may call for information or walk in for service.

If applying for My Health LA, bring photo ID, proof of Los Angeles County residence, and something that shows income like pay stub or tax forms. If photo ID is not available, an individual may provide a signed affidavit. FEES/PAYMENT SOURCE Services are provided without direct charge to the patient; Medi-Cal, Medicare are accepted; donations are requested. Eligible contract clinic patients pay no fees.
