Diabetes Screening ( Youth )

Provider: PTA TENTH DISTRICT

The program provides health services for eligible youth, in pre-kindergarten through 12th grade, who are referred by school health personnel from Los Angeles Unified School District schools. Services include BMI/body composition screening, childhood immunizations, diabetes screening, general physical examinations, health problems counseling, nutrition assessment services, optometry, pediatric cardiology, pediatric evaluation for heart disease and orthopedic disabilities, and tuberculosis screening. There are no geographic restrictions.

Vision services include eye exams and glasses. APPLICATION PROCEDURE Call to apply; a referral from an LAUSD school is required. Generally the school nurse screens the family for need as well as for the type of service. When the application is approved by the district office, the family makes an appointment at the clinic which provides the service needed. FEES/PAYMENT SOURCE Medical fees vary; vision fees are $25 for an eye exam and $60 for glasses. Financial assistance may be available.
