This community clinic provides general health services to low-income people in Los Angeles, including those who are undocumented. Services include a community clinic, contract clinic services, and diabetes screening and treatment. Services are targeted but not restricted to the Hispanic/Latino community. There are no geographic restrictions.

The clinic is a My Health LA Community Partner Clinic and provides application and enrollment in the My Health LA plan. My Health LA covers low income individuals (age 6 to adults) in Los Angeles County that do not have health insurance and cannot qualify for Medi-Cal.

APPLICATION PROCEDURE Walk in for service.

If applying for My Health LA, bring photo ID, proof of Los Angeles County residence, and something that shows income like pay stub or tax forms. If photo ID is not available, an individual may provide a signed affidavit. FEES/PAYMENT SOURCE There are fixed fees; low income patients are charged on a sliding scale.
