Provider: HERITAGE CLINIC AND THE COMMUNITY ASSISTANCE PROGRAM FOR SENIORS

The program provides services for older adults ages 55 and older who have Alzheimer's Disease, Parkinson's Disease, have suffered a stroke or suffer from a related dementia in the San Gabriel Valley, Los Angeles, Long Beach and Palmdale areas. Age restrictions apply; most services are restricted to San Gabriel Valley, Los Angeles, Long Beach and Palmdale residents.

Services include adult social day care, case management, mutual support groups, and disease/disability information. The Supportive Services Program (SSP) strives to enable older adults to remain independent and to prevent premature or unnecessary institutionalization. Individuals must be medically stable and not be a danger to themselves or others. People who use canes, walkers, or wheelchairs may participate if they are able to walk or transfer from a wheelchair with minimal assistance. APPLICATION PROCEDURE Call for information or to apply for services. An appointment is required. FEES/PAYMENT SOURCE The fee for day care is $65 per day; scholarship assistance may be available. There is an additional charge of $10 per half hour for extended day care. Transportation to the program site is by donation. SCAN insurance is accepted. Medi-Cal and Medicare do not cover this service. All services are pre-billed monthly.
