The center provides a variety of basic health services for people who live in the East San Gabriel Valley. Services include Child Health and Disability Prevention (CHDP) services, family planning, pediatrics, immunizations with physicals, and TB Mantoux tests if qualified. The center serves people who live in Covina, Hacienda Heights, Industry, La Puente, Rowland Heights and West Covina.

Contraception and reproductive health services are provided. Low-income individuals may qualify for services under the State Family PACT program. People whose income is less than 200% of the Federal Poverty Level are eligible. Services under Family PACT include a complete physical examination, STD testing and treatment, and contraceptive counseling and services. The Family PACT program serves females from 12 to 55 years, and males from 12 to 65 years. Reproductive health services include colposcopy; case specific pregnancy termination; mammography; and pap smears.

CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.

Immunizations such as Hepatitis A and B; Measles, Mumps and Rubella; Influenza; Diptheria, Pertussis, Tetanus; Pneumococcal Polysaccharid and Poliovirus, may be available to adults, age 19 and older with the physical exam only; however, there is limited availability at DPH and DHS clinic facilities. Call for information about vaccine availability. APPLICATION PROCEDURE Call for information, to make an appointment, to verify that the center serves the caller's address, or to be directed to the appropriate center. FEES/PAYMENT SOURCE There are fixed fees with the service provided and average $60 per clinic visit. The center accepts Medi-Cal, and has a variety of payment plans, including ATP (Ability to Pay), a sliding scale plan which begins at $0 for very low-income families. Medi-Cal and Healthy Families are accepted.
