Provider: LOS ANGELES COUNTY DEPARTMENT OF MENTAL HEALTH - GENESIS - FCCS OLDER ADULT PROGRAM

The program provides services for older adults age 60 and older, who are frail and/or isolated in their homes and live in Los Angeles County. Services include case/care management for frail elderly, including assistance for people who are clutterers/hoarders; geriatric assessment; medical social work and mobile health care services. The program targets frail elderly who are isolated and unable to leave their homes. There are age and residence restrictions.

The multi-disciplinary team of medical and mental health professionals visit the person in their home following a report from an agency or a call to the Adult Protective Services hotline. The team performs comprehensive health and mental health assessments. Medical and psychiatric social workers provide linkages to other support services such as relatives, church, health providers, short term counseling or whatever is needed. Family is then involved with decision making, while trying to respect the client's autonomy and independence.
