Home Delivered Meals ( People With Chronic Illnesses )

The program provides home-delivered meals for people of all ages who have mobility problems and are living with chronic illnesses who are homebound. There are no geographic restrictions.

The program delivers frozen meals and then transport them to enrolled client's homes. Meals are available to accommodate vegetarian diets or special medical needs.

Individuals who have one or more of the following chronic conditions are eligible for this program:

- HIV or AIDS, especially with an active viral presence or low CD4 count.
- Cancer, undergoing treatment.
- End stage renal disease, advanced chronic kidney disease, or serious liver disease.
- COPD or other respiratory disease requiring 24-hour or frequent oxygen.
- Advanced congestive heart failure, recent stroke or heart attack.
- Uncontrolled diabetes with A1c indicator over 8.
- Serious neurological or muscular disease such as Alzheimer's, Parkinson's, or ALS.
APPLICATION PROCEDURE Call or visit website for intake. People must provide proof of diagnosis of chronic illness(es) from their medical providers. The project accepts referrals and intakes from physicians, social workers, and significant others.

To register a complaint, call the Client Service line or download the agency's grievance form from the website and mail it to the agency. FEES/PAYMENT SOURCE There are no fees for services. A new application must be submitted and approved for each year.
