Home Delivered Meals ( People With Chronic Illnesses )


The program will assist people with COVID-19, however it is determined case-by-case and services are sometimes not available immediately. There are no guarantees that services will be provided to people with COVID-19; priority is given to people with chronic illnesses.

The program provides home-delivered meals for people of all ages who have mobility problems and are living with chronic illnesses who are homebound. There are no geographic restrictions.

The program delivers frozen meals and then transport them to enrolled client's homes. Meals are available to accommodate vegetarian diets or special medical needs. APPLICATION PROCEDURE Call for intake. People must provide proof of diagnosis of chronic illness(es) from their medical providers. The project accepts referrals and intakes from physicians, social workers, and significant others.

FEES/PAYMENT SOURCE There are no fees for services.
