The agency provides end of life services for people in Los Angeles County. Services include bereavement support groups, hospice care, in home hospice care, and undesignated temporary financial aid for families/friends of the terminally ill. There are no geographic restrictions.

In home hospice services for adult and pediatric patients with life-limiting illnesses include services in which there are medical components to the care. To qualify, the patient must have a prognosis of less than six months of life. The patient and family must agree to care that is palliative rather than curative. There are no geographic restrictions.

Hospice services include intensive palliative care for people with a variety of terminal conditions, including AIDS, Alzheimer's's disease, cancer, heart disease, kidney disease, lung disease, liver disease, MS, Lou Gehrig's disease, and stroke. The program utilizes a multi-disciplinary team of medical, social work and mental health professionals and the services of volunteers. Hospice workers travel to where the individual is living. After-hour care is available.

The bereavement support groups are open to any one in grief, and meet weekly.

The Foundation for End-of-Life Care, a non-profit entity, provides financial assistance for low-income people who are Medi-Cal eligible, or who are already hospice patients, including people receiving care from another hospice organization. A special needs program provides assistance with extraordinary services such as travel expenses for distant relatives, or purchase of special equipment such as room air conditioners to ameliorate a dying persons discomfort. Payments are made to the service provider, not to the patient or to the hospice caregiver. APPLICATION PROCEDURE A referral from a doctor, verifying the terminal nature of the patient's disease is needed for hospice services. People interested in the support group should call before attending.

Application for financial assistance through the Charitable Fund is made by recommendation of the hospice worker, completion of the Assessment form to be faxed to the Foundation at the designated number. Assessment forms are available on request at the Foundation or may be downloaded from the website www.vitascharityfund.org FEES/PAYMENT SOURCE There are fixed fees which are covered by Medi-Cal, Medicare and may be covered by some private insurance.
