Mental Health Evaluation ( Older Adults )

Provider: HERITAGE CLINIC AND THE COMMUNITY ASSISTANCE PROGRAM FOR SENIORS

The clinics provide mental health services for older adults, age 55 and older, who have Alzheimer's disease, dementia, or other conditions and/or substance abuse issues. Services include general assessment for substance abuse, individual counseling, mental health evaluation and psychiatric case management. Age 60 is the standard minimum age for these services, however, seniors who have certain conditions may be included as early as age 55. There are no geographic restrictions.

Seniors or their families may refer clients to the program or they may be referred by their therapist or physician. After an intake interview, mental health assessments, and substance abuse assessments, the client is paired with a therapist, who may be a clinician or an intern. Outpatient counseling may be arranged to take place in the client's home if the person is too frail to come to the clinic. Case management includes medication monitoring, peer counseling, and crisis intervention if needed. APPLICATION PROCEDURE Call for an intake interview. Referrals are preferred, however, families may self-refer. FEES/PAYMENT SOURCE Fees are set on a sliding scale; Medi-Cal and Medicare are accepted.
