The agency provides health services for people of all ages in the Northeast San Fernando Valley area. Services include community clinics; family practice medicine; internal medicine, child health and disability prevention (CHDP) exams, breast cancer screening; cervical cancer screening; childbirth education; colposcopy services; contraception; gynecology/obstetrics; health education; childhood immunizations, and pediatrics. There are no geographic restrictions.

Contract clinic services include outpatient health services at 4 contract clinic sites for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary dental care and general medical care. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients.

CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.

Health education includes breast and cervical cancer prevention, nutrition education, smoking prevention/education, prenatal and parenting education, family planning and STD education.

The Mission Hills clinic provides general health education services for pregnant African American women as part of the Black Infant Health Program - a L A County DPH program to promote infant health and reduce infant mortality among African American women. The program provides outreach, referrals, home visits, health education and childbirth education, skill building and peer support. The service area is San Fernando Valley SPA 2.

The Pacoima clinic is an on-site pediatric clinic at the Pacoima Middle School. The clinic serves children living within the school's catchment area. APPLICATION PROCEDURE Call or walk in to apply; an appointment is preferred. California ID, a social security number, proof of residence and proof of income are required. FEES/PAYMENT SOURCE Fees are set on a sliding scale that ranges from $0 to $35 and $50, depending on the poverty level; cash, Medi-Cal, Medicare and Medi-Cal HMO are accepted. Eligible contract clinic patients pay no fees.
