Prenatal/Postnatal Home Visitation Programs ( Low Income )

Provider: ANTELOPE VALLEY PARTNERS FOR HEALTH - HEALTHY FAMILIES AMERICA

The Healthy Homes program provides health education and family support services for low-income teens and adults who are pregnant or who have a newborn baby under three months and live in the Antelope Valley. Services include prenatal/postnatal home visitation programs. Services are restricted to residents of Palmdale, Lancaster and unincorporated areas in 93532, 93535, 93534, and 93523.

Visiting nurses provide services during in-home visits, and are supported by a multi-disciplinary team. Education/instruction and mentoring techniques help pregnant and parenting women take care of themselves and their child through age two. The program also provides parenting skills development, child development, child and home safety guidelines, home management instruction, nutrition education, and help to strengthen family relationships. Families are referred/linked to needed community services such as housing resources, medical care, financial assistance, counseling, child care, and emergency food.
