Senior Housing Information and Referral ( Alzheimer's Disease )

The registry is a toll-free telephone service which provides information, guidance, and referrals related to out-of-home care facilities for elderly adults age 60 years and older in Los Angeles, Orange, Riverside, San Bernardino, Ventura, and San Diego counties, and the greater Sacramento and San Francisco Bay areas. Services include referrals to assisted living facilities, nursing facilities and adult residential care homes. Information about facilities that accept people who have Alzheimer's disease is also available. The agency DOES NOT REFER to HUD housing or Section 8.

The program staff are experienced in the out-of-home care field and work with the caller to assess the need for placement; the type and level of care that is appropriate; and the facilities that might meet the individual's physical, emotional, financial, and geographic needs. The caller then investigates the facilities suggested and may call the program for additional referrals, if necessary. Efforts are made to locate facilities which are geographically close to the significant others of the person who is to be placed.

The registry is free to the public and lists resources for all income levels, excluding Medi-Cal. Facilities must apply to be considered as referral resources by the registry; all must be fully licensed, must complete a detailed application form, must pass an unannounced inspection by trained registry staff, and must continue to pass inspections. The registry does not officially accept complaints regarding facilities which are on their list, but refers people who wish to complain to the appropriate licensing agencies.
