State/Local Health Insurance Information/Counseling ( Youth )

The agency provides health insurance information for people of all ages in the Long Beach area. Services include Affordable Care Act information/counseling and state/local health insurance information/counseling. Geographic restrictions apply for some services.

The agency is authorized to conduct enrollment in the Healthy Kids insurance program for families who have children, age zero to five, who are without health insurance and are not covered by any other social or private insurance programs. The insurance program is provided by the Children's Health Initiative of Greater Los Angeles, and LA Care Health Plan (see the separate entries for details). The agency conducts workshops for interested parents and help to complete the applications, which are forwarded to the insurers.

Certified Enrollment Counselors are available to answer questions and help individuals enroll in the Covered California Health insurance program. APPLICATION PROCEDURE Call for information or to apply. Call the agency's insurance enrollment hotline number to find out about enrollment in Healthy kids.

Documentation requires proof of family income, including previous year's income tax form; proof of residence, such as a utility bill, rent receipt or driver's license. The person applying may be the parent, step parent, guardian or relative care giver. FEES/PAYMENT SOURCE There are no fees to enroll; Healthy City premiums are based on the number of children enrolled.
