The program provides services for people with disabilities in the Pomona Valley. Services include caregiver support group for informal or kinship caregivers of stroke victims; health/disability related support group for victims of stroke; independent living skills instruction for stroke victims; and stroke rehabilitation. There are no age restrictions; the service area is Pomona Valley.

The group is called 'Finding Your Way Back, and meets one day per week for four hours. The program provides individual health problems counseling, group support, re-training, socialization skills and training in daily living skills. The retraining area is equipped with computers and includes therapy for aphasics. The group is professionally led by an instructor, and credentialed re-training specialists. It includes topics for caregivers as well as for people who have had a stroke. APPLICATION PROCEDURE Call the center for information. FEES/PAYMENT SOURCE There are no fees for service.
