The hospital provides outpatient health services for people in Los Angeles County. Services include contract clinics, family and community medicine, health education, occupational medicine, physical therapy, sleep disorders clinics, sports medicine and urgent care centers. Services are provided at four locations. There are no geographic restrictions.

Contract clinics services include outpatient health services for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary general medical care. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients.

The Pomona Valley Health Center - Claremont provides community education, family medicine, imaging, occupational medicine, physical therapy and other rehabilitation services, a sleep disorders clinic and Urgent Care services. APPLICATION PROCEDURE Call or walk in for service; appointments are preferred. ID and proof of income are required for income-eligible programs FEES/PAYMENT SOURCE There are fixed fees; there are no fees to eligible contract clinic patients.
